To make things easier for all, here is the Request to Stay Late Form. Remember, it needs to be handed in to the Main Desk by 5 PM Wednesday, April 29, 2009.

To download the above form: Simply double-click on it to enlarge it. When it appears fully on your screen in its large size, simply right-click to "save as" (or for you Mac users, just drag it to your desktop) and then open it. Before printing, preview it to make sure the page is oriented correct and then print.
